Support Groups - Patients, families and caregivers have the opportunity to
attend on-site support group sessions. One-on-one meeting with social workers, psychologists
and other support personnel may also be provided.
Family Conferences - Patients and their families may meet with members of
the treatment team to discuss progress, discharge planning and any
concerns.
Family & Caregiver Training - Training is provided for family members
and caregivers to ensure the skills necessary to assist the patients with daily activities
in the home or other environment. This promotes the continuation of the rehabilitation program
after discharge. Family members and caregivers are encouraged to attend the New Start
stroke education classes offered in conjunction with the American Heart Association.
Home Assessment - Prior to discharge, a staff member may evaluate the
patients living environment to assess the ability to function in that setting.
Recommendations for any adaptations are also suggested.
Continuum of Care - After discharge, follow-up appointments are scheduled
with the primary care physician or specialist, and the rehabilitation physician. This helps
to ensure that the patient is receiving the recommended home health or outpatient services
and provides the continuity necessary for successful rehabilitation. The Rehabcentre also
offers on-site outpatient physical, occupational and speech therapy.
Program Outcomes - The Rehabcentre shows positive results* - with
patients obtaining the independence skills necessary to return to the home environment.
This helps demonstrate the success of our rehabilitation program, as well as the satisfaction
of our patients.
