Patients take part in a minimum of three hours of therapy per day. Patients
at the Rehabcentre may also participate in the following services:
Community Outings
The reintegration into society is an important component in the rehabilitation
process. Patients participate in activities that facilitate mobility, safety
and judgment, and also challenge their communication skills. Activities may
include grocery shopping, restaurant visits and other community
events.
Family Conference
The patients and their families may meet with the treatment team in a formal
conference to discuss progress, discharge planning and any
concerns.
Family / Caregiver Training
The training is provided for family members and caregivers to ensure the skills
necessary to assist the patient with daily activities in the home or discharge
site. This promotes the continuation of the rehabilitation program after
discharge.
Support Groups
The patients, families and caregivers have the opportunity to participate in
on-site support group meetings. One-on-one meetings with social workers,
psychologists and other support personnel may also be provided as
needed.
Home Assessment
Prior to discharge, a staff member may evaluate the patients living
environment to assess his or her ability to function in that setting and to
make recommendations for adaptations.
