What does my case manager do?
Your case manager is your resource to assist in coordinating your care, communication and transition back to the community. The case manager will act as a liaison to relay information between the rehab Team, you and your family. She or he will often assist in obtaining information about your insurance benefits to arrange for payment when services or items are covered by your health plan.  

How long will I be in the Rehabcentre?
The length of time spent in the Rehabcentre depends upon each patient’s individual, medical needs. The Team members will work with you shortly after admission to the program to set a tentative discharge date. This date may change based upon your rate of progress. Discharge from the program depends upon meeting functional goals, medical stability and safety: usually this occurs when a patient no longer requires or is able to benefit from the 24 hour support and nursing care, daily physician visits and intensive therapy program.  

What happens after I complete my program?
The next phase of each patient’s care is based upon their clinical needs. Many of our patients leave the program and go home where they continue to receive care from visiting (Home Health) therapists. If appropriate, our “graduates” may also continue to receive care as outpatients of the Rehabcentre. Occasionally, some of our patients may require more extensive therapy that can be provided at a skilled nursing facility. Other discharge destinations that we may recommend include “transitional living” or even day treatment programs. The specific recommendation for you will depend upon your needs, support at home and resources. The team will work with you to identify the optimal choice.

What do I need to bring to the Rehabcentre?
Bring your own clothes. In “rehab” you will be wearing regular clothes — not hospital gowns. Since you will be exercising, loose-fitting clothing such as sweat suits, and tennis shoes or sturdy lace-ups are preferable. Please bring what is comfortable for you. Three changes of clothes are recommended, and laundry will be your family’s responsibility. Also, please feel free to bring personal items such as a toothbrush, razor or anything else that will help you feel at home.
